Early pregnancy and general gynaecology scans can be performed either trans-abdominally (tummy) or trans-vaginally (internal). Both use ultrasound (sound waves) in the same way but approach the area they are looking at from a different angle. Each method is completely safe for pregnant and non-pregnant women. Both methods send sound waves back to the computer in the ultrasound machine, which then produces an image on the screen.

A trans-abdominal scan needs a full bladder as it pushes the uterus higher in the pelvis. The ultrasound waves pass through the bladder see the uterus and baby far clearer. In most cases this type of scan is suitable for women over 8 weeks pregnant. In women with a BMI in normal range, a baby may be seen under 8 weeks. Women with a higher BMI may see less clear images.

If a pregnancy is less than 8 weeks, a vaginal ultrasound may be offered to see clearer images. The bladder needs to be empty so the uterus stays lower in the pelvis and closer to the vaginal probe. The sonographer will always provide a clear explanation and consent from you before the scan. The probe is covered and only inserted about 3cm into the vagina.  Privacy and dignity is maintained, not only during the scan but when you are getting ready. This type of scan isn’t painful but can just feel a bit odd. It is perfectly safe and will not cause any problems for the baby or cause any bleeding to happen. This can be the best way to see a tiny baby, but also the womb lining or ovaries for women having fertility treatment.

Women who have had both types of scan often say how much easier it is to have an internal scan. Drinking a lot of water if you are feeling sick isn’t easy. A popular comment is the relief of not having someone pressing on a very full bladder! However, remember you are in control of how your scan takes place and can you can choose to be scanned either way. Some couples are very happy to know their baby is ok, seeing a heartbeat and to take home slightly ‘fuzzy’ photos. Whereas others might want an internal scan to obtain the best quality images available. The choice is yours, and the sonographer will help you decide which is best for you.
